- refactors QuestionOption 'other' options can have custom text
- removes thumbs up/down buttons (an old proof-of-concept not used by any forms)
- adds validations for each type of question, so answers conform to question options
* fix: remove comma from submission.rb * add question count and form kind to form show page * update footer link * add top margin about answer field tag in the form builder * update dockerfile * update gems
