What I understand is: If the computer and Bluetooth terminal are both Apple devices, the MAC address will be resolved to a fixed real (public) address
However, when I am using it, when my AppleWatch Bluetooth is disconnected, BLEUnlock cannot automatically connect to the AppleWatch and I need to manually select it in the list. What is the reason and how to solve it?

I don't really understand how it works, but sometimes if you try to connect your device and Mac in System Preference > Bluetooth, it would resolve public address.

Fortunately, on Apple devices, if you are signed in with the same Apple ID as your Mac, the MAC address is resolved to the true (public) address.
